This Privacy Policy sets out how Apricity Health LLC (Apricity Health) uses and protects any information that you provide though the ApricityCare app and the Apricity Health Oncology web portal. We are committed to ensuring that your privacy is protected. Apricity Health may change this policy from time to time by updating this page. You should check this page from time to time to ensure that you are happy with any changes.

Definitions:

·        ApricityCare app: The mobile application for the patient that will allow you to share symptoms and details directly with your doctor's office. 

·        Apricity Health Oncology web portal: The provider web portal gives the doctor's office a tool with which to receive information directly from the patient then make assessments and do a work up with ApricityCare Oncology recommendations provided.

·        Services: Apricity Health Platform as a service allows us to take PRO (Patient Reported Outcome) data coupled with the curated SME (Subject Matter Expert) knowledge and expertise to build timely recommendations for the provider while educating the patient.

·        Consultations: You will be invited to have regular visits with your provider based on the information collected by the ApricityCare mobile app to both confirm the results and respond quickly to symptoms flagged as critical.

·        PHI: Patient Health Information under US law is categorized as any information about health status, provision of health care, or payment for health care that is created or collected by a Covered Entity (or a Business Associate of a Covered Entity), and can be linked to a specific individual. This is interpreted rather broadly and includes any part of a patient's medical record or payment history. 

What We Collect

Apricity Health will collect the following information from you when you register for an account with ApricityCare (“Account”) or use the Services: 

Registration and Health Information
When you register for an Account, in order to identify you properly and contact you when needed, we will collect identification and contact information, such as name, email address, mailing address and demographic information.

When you use the Platform, we will collect and store personal health information, such as symptoms, medical history, appointment history, diagnosis and treatment information.

Log and Website Information
When you access and use the Services, we may automatically collect certain technical information about your visit, including the date, time, browser type, your internet service provider, your IP address, device information (including device identifiers), geo-location information, computer and network performance data, the URL that you are coming from and your navigation history in order to customize and personalize your experience on the Services, improve our Services and for statistical research purposes.  The log and website information we collected will not be shared to any external parties. 

Information from Consultations
Data obtained from Consultations will be anonymized and de-identified before being analyzed and used by Apricity Health for product development and research purposes.

Use of Your Information

Consultations and Healthcare Services
We will use the personal information and PHI that we collect from you to schedule, personalize and improve the effectiveness of your Consultations and update your patient record.

We may disclose your personal information, including PHI to third-party healthcare professionals involved in providing you with healthcare services, such as your physician, specialist, pharmacist, nutritionist or lab technician. We will only disclose your personal information to a third-party healthcare professional, not associated with Apricity Health, after obtaining your consent to the disclosure. Only the healthcare professionals involved in providing you with healthcare services can access your PHI.

Apricity Health can share with selected third-parties demographic and contact information about me (including but not limited to: name, DOB and any email addresses or phone numbers), for reasons including but limited to: verifying your identity, scheduling appointments, confirming appointments, customer support and technical support. 

Business Matters
We may disclose and share some information to explore and/or undertake a corporate transaction, including a merger, acquisition, IPO, reorganization or sale of Apricity Health. Your personal information relevant to the transaction, can be used and disclosed solely for the purposes related to the transaction and will be protected by security safeguards appropriate to the sensitivity of the information.

Your PHI will not be disclosed and will remain confidential.

Security and Compliance

We are committed to ensuring that your information is secure. In order to prevent unauthorized access or disclosure, we have put in place industry standard safeguards to protect personal information, including electronic and managerial procedures to safeguard and secure the information we collect online. 

In association with the software, content, and services that we provide to our clients and partners in the healthcare industry, we have policies and procedures to address the privacy and security implications of the rules promulgated pursuant to the Health Insurance Portability and Accountability Act of 1996 (HIPAA).

Apricity Health reserves the right to disclose personal information with third parties if required to do so by law; or if we believe such action is necessary to comply with legal process served upon us; protect or defend our legal rights or property; or to investigate, prevent, or take action regarding illegal activities, suspected fraud, potential threats to the physical safety of any person; or violations of any obligations owed to Apricity Health.

Apricity Health reserves the right to change the terms, conditions, and notices under which ApricityHealth.com is offered, including but not limited to any changes associated with the use of the ApricityCare app, website and this privacy policy now or in the future.   We will display the date this document has been updated at the top of the page. Also, we will ask you to accept this document again on login in the event we make any changes. 

Controlling Your Personal Information

You can stop being in the research study at any time. Tell the research doctor or your doctor's office if you are thinking about stopping or decide to stop. Leaving the research study will not affect your medical care. You can still get your medical care from your doctor.
